Updated interment/entombment procedures with area Funeral Homes

At Greenwood Memorial Gardens, we, like you, are committed to serving each family with compassion & professionalism. It is imperative that we honor their privacy and legal rights, while remaining compliant with appropriate local, state, and federal regulations. Therefore, we have updated our interment/entombment authorization procedure, effective immediately.
• The legal next of kin, or their designated legal representative (Authorized Representative) will continue to sign the interment authorization.
• At the time of signing, a valid government-issued ID will be required and a copy of which will be retained for our permanent record.
• A copy of this letter will be provided to the authorized representative.
• Greenwood Memorial Gardens will not release any personal information from our files without the explicit permission of the authorized representative.
• If the authorized representative wishes to provide verbal permission for the release of information, we will contact that individual directly to obtain and document that permission.
Required Information at Time of Notification of Death:
• Deceased full legal name
• Deceased date of birth and date of death
• Full legal name of the authorized representative and phone number
• Property owner name(s)

• Type of service (Church, Chapel, or Graveside)
• Requested interment/service date/time *Service Time (9am, 11am, 1pm & 3pm) • Make & Model of the outer burial container being utilized.
Understanding the vault provider provides grass tents and chairs with sufficient personnel to work the service.
Set up must be completed no later than 2 hours prior to scheduled arrival time.
Scheduling of the interment/entombment.
When requesting a day & time of service, please remember to refer to our online Google Calendar that we have shared with area funeral homes to ensure availability and avoid scheduling conflicts.
*Please note however, that scheduling of interments/entombments are not finalized until the necessary authorizations are completed and any fees due are paid. Please do not finalize services with churches/clergy, nor publish days/times in obituaries and/or websites until confirmed with cemetery staff. Interment authorization still must be completed with the authorized representative in person, at the cemetery, no later than 48 hours prior to the requested time. In very rare occasions, an accommodation may be made for electronic signatures and payments, which will be addressed on a case-by-case basis at the sole discretion of the cemetery.
Only after the authorization is signed and any fees paid, will the interment/entombment schedule be finalized.
